Thomas Bleasdale M.D.
Dr. Thomas Bleasdale graduated from Rush Medical College in 1998. After completing his residency at Rush Presbyterian St. Lukes Medical Center in 2001, he served as Chief Resident for the department of Internal Medicine in 2002. He is board certified in Internal Medicine. Dr. Bleasdale’s interests include preventative medicine and patient education. He currently resides with his family in Arlington Heights.
Donald Cyborski M.D.
Dr. Donald Cyborski graduated from the University of Illinois Medical School in 1976. After completing his three year residency at the University of Illinois in 1979, he worked in the Emergency Department at Northwest Community Hospital until 1981 and has since practiced Internal Medicine in the community. He is board certified in Internal Medicine.
Jong Ho Ham M.D.
Dr. Jong Ho Ham graduated from the University of Illinois Medical School and completed his residency training at the University of Illinois at Chicago Hospital. He is fluent in both English and Korean. Dr. Jong Ho Ham is board certified in Internal Medicine
Theodore Morrison Homa MD CMD
Dr. Theodore Morrison Homa is a graduate of St. Louis University School of Medicine. His post-graduate training in Internal Medicine was at the University of Connecticut. He is board Certified in Internal Medicine and is a Certified Medical Director in long term care. He has been practicing since 1977 in Internal Medicine, geriatrics and long term care. As such, he is one of the group’s physicians treating patients in skilled care nursing facilities as well as outpatients in a campus-based clinic on the grounds of the Lutheran Home in Arlington Heights. Dr. Theodore Morrison Homa is dedicated to carefully listening to patients and their families in order to provide the highest level of care and satisfaction.
Steven Kim M.D.
Dr. Steven Kim grew up in the northwest suburbs and has been serving the same community since he finished his training. He did his undergraduate work at Stanford University in California. He graduated from medical school and completed his residency at the University of Illinois at Chicago. Dr. Steven Kim is interested in geriatrics, and goes to several extended-care facilities in the area. Dr. Steven Kim and his wife enjoy traveling.
Andrew Krock M.D.
Dr. Andrew Krock graduated from the Honors Program In Medicine at Northwestern University in 1984 and completed his residency in Internal Medicine at the Mayo Clinic in 1987. Dr. Andrew Krock is board certified in Internal Medicine and holds a fellowship in the American College of Physicians. A life-long resident of Arlington Heights, he is married and has 3 children.
Stephen F Marshall M.D.
Dr. Stephen F Marshall received his Doctor of Medicine degree from Chicago Medical School in 1967 and trained in Internal Medicine at Northwestern University. Dr. Stephen F Marshall completed a subspecialty fellowship in Gastroenterology, also at Northwestern, and is board certified by the American Board of Internal Medicine and the American Board of Gastroenterology. Dr. Stephen F Marshall resides with his family in the northwest suburbs.
Marvin Peiken M.D.
Dr. Marvin Peiken graduated from Northwestern University Medical School in 1975 and finished his Internal Medicine residency at Northwestern in 1978. Dr. Marvin Peiken is board certified in Internal Medicine. He is the Medical Director of Northwest Community Hospital’s home care department and is also board certified as a home care director.
Denise E. Rouse PA-C
Denise E. Rouse was a National Merit Scholar at Northwestern University from which she graduated with a BA degree in mathematics. Denise E. Rouse graduated from Midwestern University with a Masters of Medical Science degree in Physician Assistant Studies. Denise E. Rouse is a lifelong resident of the Chicago area and resides in the northwest suburbs with her husband and daughters.
Ghanshyam Shah M.D.
Dr. Ghanshyam Shah graduated from the University of Illinois Medical School in 1995 and went on to complete his residency training at the University of Illinois at Chicago medical center in 1998. Dr. Ghanshyam Shah is board certified in Internal Medicine, is married and resides with his family in the northwest suburbs.

Dr. Hetal Shah graduated from NHL Medical College, India, and completed her Internal Medicine residency training at St. Joseph Hospital in Chicago. Dr. Hetal Shah is board certified in Internal Medicine. Dr. Hetal Shah has practiced Internal Medicine in the northwest suburbs since completing her training and currently resides with her family in the area.
Catherine Wood M.D.
Dr. Catherine Wood was born in St. Louis, MO and was raised in Dayton, OH. Dr. Wood attended medical school at Rush Medical College. After completing her residency in Internal Medicine at Lutheran General Hospital, Dr. Catherine Wood has practiced in the northwest suburbs since 2007. Dr. Wood and her family reside in the Arlington Heights community. Her medical focus is on preventative medicine and patient education. Dr. Wood is Board Certified in Internal Medicine.
Leo Kanev MD CMD
Dr. Leo Kanev graduated from Rush Medical College in 1998. He completed Family Medicine residency at Lutheran General Hospital in 2001 and has been in solo practice in Buffalo Grove until September of 2012 when he joined Northwest Suburban Physicians. Besides seeing patients in 800 Oakton office, Dr. Kanev is able to visit patients in their homes if the need arises. His other interests include advanced direction of rehabilitation services after hospitalization and care of medically complex patients outside hospital environment. Dr. Kanev lives with his family in Evanston. He is working hard to continue improving his tennis game.
Dr. Madhu Damera
Dr. Madhu Damera graduated from MR Medical College, India in 2000. He completed his Internal Medicine residency at Aurora Sinai Medical Center in Milwaukee in 2004 and subsequently worked in the Elgin area primarily as a hospitalist but also as a primary care physician until 2012. He is now our hospital based physician. He lives in Arlington Heights.
